Acknowledgement and Acceptance:

By signing the Registration Form, the registrant (parent or legal guardian) acknowledges and accepts the following Refund Policy, Privacy Statement, and Consent and Release Statement.

Refund Policy:

The registrant must complete a "Request for Refund" form. Upon acceptance of the request by the Treasurer, there is a 4 - 8 week processing period. There will be no refunds after November 15th. No refund will be issued until any equipment, or property, of St. Marys Minor Hockey Association has been returned and satisfactorily accepted by St. Marys Minor Hockey Association. If the request is accepted by Sept 1 - 100%; November 15th - 75% of the registration fee less insurance will be refunded. The insurance fee for this year is $51 per player. There will no refund of "Booster Draw Tickets" once they have been distributed to the teams. Exception to the Policy: Any midget boy who joins a junior team, within OMHA, a full refund of any fees will be refunded provided the junior team pays SMMHA the minimum as set in OMHA Regulation 11.0 "Development (Release) Fees.

Privacy Statement:

All information collected by St. Marys Minor Hockey Association will only be used for the administration and operation of the Association. Certain information is given to other operational organizations (Hockey Canada , Ontario Minor Hockey Association, Ontario Womens Hockey Association, and Ontario Hockey Federation), which governs hockey within Ontario, which St. Marys Minor Hockey Association is an active member.

Consent and Release Statement:

Upon acceptance of this registration application, the registrant consents that the players named on this application are able to participate in the hockey program of St. Marys Minor Hockey Association. The registrant agrees to abide by all regulations, rules, bylaws, policies, and the Code of Conduct of St. Marys Minor Hockey Association. The registrant agrees to release St. Marys Minor Hockey Association from all claims for damages from any accidents, or injuries, which is caused by, or arises from, participation during any game, practice, or function of the program.
