All OMHA and OWHA team Trainers are required to complete the Hockey Canada Injury Report when a player injury occurs on or off the ice during a team hockey event (game, practice, other) and player requires medical attention and/or hospitalization. If this occurs and after the HC Injury Report is submitted, the Trainer must notify SMMHA using the SMMHA Injury Notification form.
